This includes managing and administrating medication, post-surgical care, therapies and more.
In-Home Telemonitoring
In-home telemonitoring is vital for the patient’s recovery and care management. Our team of nurses remotely monitor your condition on a regular basis. This can include your vitals, such as blood pressure, pulse, weight and more. Telemonitoring is done remotely minimizing trips to the doctor or hospital.
We strive to educate our patients and their families about the recovery plan and health management. We report your results to your primary doctor so that he or she can recommend the right treatment. When any changes to your health or vitals are reported, a skilled nurse may go to your home for a follow up visit. If you do quality for remote telemonitoring, we strongly recommend this added benefit for both the patient and the family.
SN, PT, OT and ST Home Care
As a big part of our in-home care program, we also extend therapy care services for in-home patients. We know it can be difficult as you recover from your illness. However, our therapists are trained at working with you at a pace that is effective for your progress.
